Forms
The first step for those interested in traveling with Discover Southern Italy
is to complete the application form. The application form is a pdf, which
can be filled in on your computer, saved, and printed. Or open the application form, print it and
complete it by hand. Mail the application to
me at the address near the top of the form. Please do not send a deposit
with your application. A
registration form will be emailed after your application has been reviewed and
space is available.
Click Application Form to open the
application form. This form will load slowly on some systems. If you
are filling in the form on your computer be sure to save the form as you fill out the information and note the
location to which it has been saved. Adobe Acrobat Reader
version 7 or above is required to fill out the form on your computer.
It can be opened and printed with earlier versions of Acrobat Reader.
A
Deposit in
the amount of $500 is due by November 30, 2009 and the final payment of the
balance is due by February 20, 2010.
Cancellation and
refund: After Feb 25, 2010: 30% penalty. After March 05, 2010: 50%
penalty. After April 15, 2010: 75 % penalty. After April 25, 2010
there will be no refund.
Payments will be
made by credit card, check or money transfer to the Friendship Force
International office in Atlanta, Georgia.
